Affidavit of Enrollment — Choice Academe
I, the undersigned parent or legal guardian, affirm that the information provided in my child’s enrollment application is true and accurate to the best of my knowledge.
I understand that Choice Academe provides afterschool care and tutoring for children Pre-K through 5th grade, with a balance of structured activities and free play, including homework help.
I agree to the program’s pick-up procedures, and I authorize only the individuals I have listed to pick up my child. I agree to keep my contact and emergency information current.
If using CHOOSE Act education savings account funds, I affirm that my student is a participating student under the CHOOSE Act and that funds will be applied only to qualified tutoring expenses.
